What to Bring

To help us better understand your health and needs, we'd ask that you please bring the following to your first visit with us:

X-Rays

Any current x-rays or CT scans from your referring dentist (in most cases this can be forwarded to us). If it's been more than 2 years since you last had x-rays, we'll be happy to take them for you during your initial consultation.

Completed Questionnaire

Complete our medical/dental questionnaire prior to your first appointment. You can find the Questionnaire here. If you aren't able to complete the form before your visit, not to worry, you can fill it out at our clinic.

List of Medications

Bring a list of any medications you are currently taking.

Dental Insurance Information

Please bring any forms or insurance cards.

What to Expect

During your initial examination Dr. Bouclin and our team will:


Step 1

Review your in-depth medical and dental history.


Step 2

Measure your oral health and evaluate any bone loss, loose teeth, bite and signs of periodontal disease.


Step 3

Perform an oral cancer screen.


Step 4

Perform a full muscle and jaw joint evaluation.


Step 5

Take digital photographs.


Step 6

Take x-rays (if necessary).


Step 7

Take impressions (if necessary).


After completing this assessment, Dr. Bouclin will evaluate his findings and discuss next steps and possible treatment options, working with you to create a customized plan to help you reach your health goals. In some cases, the treatment plan may require coordinated care with your family dentist or another specialist, which would also be discussed during your initial consultation.

Office Policies

Cancellation Notice

If for any reason you need to cancel your appointment, we ask that you provide 48 hours notice to avoid any additional fees.

For patients Under 16

For the purposes of legal consent and authorization, a parent or legal guardian must be present at a consultation involving minors, and will be required to sign patient records.

Latex Free Environment

Our clinic does not have any latex gloves or use any other materials which are know to have latex in them.

Scent Free Policy

To provide a safe environment for patients and staff, we have a scent-free policy and ask that you refrain from wearing any heavy perfumes, colognes or items with a strong scent.
